Happens all the time. When I first moved here, I worked with a woman who was 23 and had a 4yo. Previous 3 pregnancies had ended with an abortion. She flat out told me that she 'loved' her son, but wished she hadn't decided to carry him to term. Definitely did not want anymore children, and from talking to her, I suspected that it was a great idea for her not to.
We all had an HMO, so choices of doctors were limited. Her GYN told her that he was 'pro-life' and couldn't tie her tubes, even though she told him that if she got pregnant again, she'd have another abortion.
My mom had 5 kids in less than 9 years. She went to her parish priest in tears because she couldn't handle the kids she already had - priest literally said "Well, if you don't want kids, then why are you having sex in the first place?" OB/GYN who delivered me told her that since she was Catholic, that birth control was 'clearly' out of the question. Total assholes.
And for those who doubt that this shit can happen today - look at Kavanaugh's view on contraceptives. He agree with Priests for Life that their view of contraceptives as 'abortion-inducing drugs' was valid.
