I feel compelled to point out that Iceland is a very small country, so they can whisk someone through their system fairly quickly. If you have something similar in the US, a gyno will usually see you within 48 hours and you'll get an ultrasound the same day as well.
The real issue here is the cost - that same mammo (without heaters!) and doctor's visit would probably cost a minimum of $1000 (assuming you didn't need more past the mammogram). And if you don't have insurance, it can be damned hard to find a doctor here who will see you, let alone refer you for a test without a damn good cause.
One of the things that Rally always conveniently overlooks in his story of his robotic surgery is that if he hadn't had top-notch health insurance he would never have been told it was an option for him - doctors tend not to recommend treatments that they feel their patients can't afford.

We all paid in to rally's surgery. Almost nobody can actually afford American charges. Great for making profit, not so great for health care delivery.

I will disagree with you on this - Rally's surgery was covered by his health insurance, which happened to be a top-of-the line plan.
What we are paying for are the thousands of people who can't afford good insurance, or who have no insurance at all and end up in the hospital or ER without the means to pay. Because those costs get passed on in the form of higher prices and personal bankruptcies. Because there is no free lunch.
But it's all okay, because the Rally's of the world can get state-of-the-art treatment.
